Cover frames: what Instagram actually stores
When a video is uploaded, Instagram keeps a still image beside it and serves that as the cover. It is a real JPEG with its own dimensions, usually matching the video's aspect ratio — 1080×1920 for a vertical Reel when the upload was large enough.
The version you see in a feed or a grid is a much smaller rendition of that same still, because a scrolling page cannot afford the full file. That distinction is the whole point of this tool: it returns the stored cover rather than the display-sized copy.
Why a screenshot of a Reel is not the cover
A screenshot captures your screen. You get whatever your display was showing, cropped to your viewport, re-compressed by the screenshot format, and often with interface elements baked into the picture.
A frame grab from the video is not the same thing either. The cover is a separate image the creator or Instagram selected, and it is frequently not identical to the first frame of the clip — so pausing at the start and capturing that will give you something subtly different.
When a thumbnail cannot be fetched
A lookup fails when the post is not publicly reachable: a private account, a deleted post, or an age- or region-restricted upload. Photo posts return nothing here by design, because there is no cover frame to return — the photo itself is the picture, and the photo downloader handles those.
Instagram also changes its page structure from time to time, which can break a lookup until the extractor catches up. If a link fails once, run it again before assuming the cover is gone.

Can I get the thumbnail from a private account?
No. Only content Instagram already serves publicly can be fetched, and a private account's posts are not publicly served. No tool can change that without your login, which you should never hand over.
Why did a photo post return nothing?
